Working for the state has great benefits and usually good pay. Once you get "your foot in the door" the opportunites are great. Can't really think of negatives.

omg no! you should call them if this woman has not been feeding her kid and not provieding for her like a mother should then yes you should call social services a child does not deserve to be put through all of that! the mother is oviouly not thinking about whats best for her like you said she has no health insurance, no food, and a very low percentage weight for her age you need to do something quick before things progress any further!

Well I know for a fact there are a few things that would qualify to take a child out of a home, especially if the child is in an extremely unsafe environment.
Example:

-abuse that you have either witnessed or have proof of,
-if the home or living conditions are extremely poor or unsanitary (dirty,etc..)
-if the parent or parents are abusing drugs/alcohol

etc..
but I'm not 100% on mental health. if you really feel the child is in danger, maybe you could call child services and plead your case to them. you might get custody until your sister's mental health state is better.
