We Answered:

Did she ever have a card in the other name? Provincial Health Departments sometimes have unusual naming procedures. It's usually not possible (or very difficult) to CHANGE the name, and they may require both names in order to allow both parents to deal with the child's medical needs.

Quebec specifically has different rules regarding the mother's name, so perhaps this is some offshoot of those policies.

I'm in BC, and we've essentially given up on trying to convince the BC Medical that my son has NEVER gone by his mother's maiden name (nor does she, and yes, we were married and she was using her married name long before he was born). But that's the way they have it on file, and we cannot seem to change it.

We Answered:

you must have a court order for both child support and requiring the father to enroll the child into DEERS and get Tricare(medical Inurance).

with one in hand you can contact the command and ask that it be enforced.

We Answered:

On your federal taxes yes. Any person who is provided more than 50% of their living expenses from you and is legally residing here can be claimed on your taxes.

How long after having baby will I be able to receive a birth certificate or health card ?

We Answered:

i filed for my son right after he was born and i didnt get a birth certificate til he was 5 months old, his health card was about 6-8 weeks after he was born

Yes you must visit the OHIP office.
