We Answered:

I had my son when i was 16 and i got married to his father 2 months after i turned 17 but i was only able to marry him with my moms permission. So she has to have your parents sign a paper saying she is allowed to marry him otherwise she has to wait till shes 18.

Im sure if you asked your health visitor they would issue a new 1 to you!!! The health visitor would have all relevant documents that are needed for ie 3 month checks etc
