A Durable Health Care Power of Attorney would be the place to start. Additional forms may be necessary, depending on your particular state laws.

You will need at least three notarized originals - one for you, one for your parent, and one for your parent's doctor. If it were me, I would give a fourth notarized original to a trusted relative who doesn't live with either you or your parent.

Before the time arrives that you would have to use it, make sure that you and your parent (going there together) give one of the originals to your parent's doctor, that your parent states their wishes (the ones in your question), and you confirm with the doctor that they now have all they need to grant those wishes IN ANY CIRCUMSTANCE. You need your parent to be there when the form is given to the doctor so the doctor has no doubt that your parent was of sound mind when they signed the form.

The main reason for giving an original to a trusted relative is that if you take your parent to the hospital, this relative could bring their original to the hospital if needed. You wouldn't have to leave the hospital to go get it. There's also a chance (especially if your parent lives with you) that both of your originals could be lost or destroyed, and this provides for a backup original.

If TPR, NONE and chances are the bio-parents don't care anyway. By the time TPR has happened, the bio-parents have been given so many chances...in my opinion, if TPR happens, the bio-parents are so selfish and wrapped up in their own problems and issues that they probably can't and don't think about their children.

If not TPR, they should definitely have a say in many things, UNLESS they are so abusive that they are dangerous and it is known that TPR is going to occur.

Before my bio-parents had their rights terminated, I could not have different haircuts and could not have some dental work that I needed. The dental work was actually a medical problem, but it could not be done. IN my case, they could not find my bio-mother to talk about it, so they had to wait a certain amount of time to call it "abandonment." Then TPR...and once TPR occurred, I got my teeth fixed.

However, if you are not on your parent's insurance policy now you might have to wait until the anniversary of the policy before you can be added. Have your parents call the customer service number on the back of their ID card to find out how their insurer is complying with the new regulations.

I took child development in ninth grade and i was the only student that wasnt a parent. I just wanted to take the class so i could get a babysitting job and know what i was doing. It was a great class and it taught me alot. Mostly it focused on the fact that newborns bond through feeding and the best way to feed was breastfeeding bc they get the eye contact, the skin to skin contact, and they feel the warmth and comfort of their mom everytime they eat, ultimately creating a strong attachment. They covered breastfeeding quite thoroughly, they even went so far as to show a video of mothers learning how to breastfeed, and more established breastfeeding mothers so we got the idea that it wasnt going to be easy but it was possible.

I breastfed both of my babies not only bc my mom breastfed us but bc of what i learned in that class. It wouldnt hurt to teach all students about breastfeeding, it should be basic health information. 900 infant deaths a yr could be prevented if moms breastfed for the first six months of life, thats a huge number.
