Go for the M.D. You don't have to decide on a specialty (if any) right away. Get into med school first, and make those decisions later.

Your options are limited with an R.N., and you will always be answering to someone (though you may be in charge of some folks eventually). With an M.D., you call the shots. If you want versatility, respect, money, and power, you should go for the M.D.

I also agree with Spanish.

Out of those options, that is the language that is used in more developing countries. Japan is a highly developed country, Latin is a dead language, Germany is also very well developed as well. Russia doesn't have the most amazing health care but they do a decent job.

So out of all of those if you really wanted to practice medicine for people who have poor medical infrastructure, spanish would be your best bet. Think Latin and Southern America!

Sign Language could be an option as well simply because of the added benefit of being able to talk to a group of patients who would normally require a translator. This allows you to ask better questions and get better symptoms to be able to diagnose better since your questions are not getting filtered (as much). However this would not be as beneficial as Spanish.

One big thing to remember is that if you do learn a language if you don't continue to use it at least a few times a week you will pretty much forget most of it within a few years. I took 4 years of spanish in High School and can't remember much more than to ask where the bathroom is and to exclaim my pants are on fire. This of course is much to the chagrin of my father who is a spanish teacher. =P
