What you are seeing is a 'shorthand' notation for things that are standard in the chart. One corner could be seriousness level; one corner could be follow-up in days; one corner could be prescriptions written; one corner could be lab/x-ray work ordered.

Generally, the medical staff is in a serious time crunch and limited in how long the doctor is allowed to spend with each patient. The support staff will look at that tic-tac-toe and know exactly what they need to do without having to read your file notes or try to read the doctors mind or take your word for it.
