He can't travel there, end of story...

Basically it's down to the writers, he can't cross his own time stream and alter events he's already been a part of. (Except the Three Doctors, Two Doctors and Five Doctors have already proved he can)

According to the writers of the show, some events are part of history are locked and can not be changed (Bowie Base - Waters of Mars) and others are in a flux and any outcome is possible (Hungry Earth/ Cold Blood)

doctors have offices in more than 1 location that they work at because sometimes in 1 location they dont get enough customers to be busy all the time and so they actually have 2 or 3 offices they work at.

now, traveling the world....well, there are many doctors who dedicate themselves in poor areas of africa where they don't seek pay but just want to help people.

Transfer out of community college as soon as possible. You need a lot of premed classes in order to apply to med school after getting your bachelors, but many med schools won't accept premed credits taken at community college. Take the generic classes you need to transfer as soon as you can and don't take the premed classes until you transfer.

Because the war between the time lords and the darleks is time locked and also he can not change certain fixed points in history. who decides what is a fixed point and what is not I have no idea

Home care physicians. They can act as your primary doctor or they can provide consults to your primary doctor in the event that you are unable to visit him/her.

I would suggest you ask your union. Compo is very hard, the insurance companies will try to get out of paying you for anything.
My feeling is yes you should If that is the only available appointment you could get. Good luck
