First I would report Celtic to the state insurance commission for not paying claims. But if the service you received from BCBS was better than the service you received from Celtic you have to decide if the difference in cost is worth it for the difference in service.

Only you can determine what is the best health insurance plan. I say that because you are the one that has to make the decision of what the right balance is between affordability and policy benefits.

You indicate that you are reasonably healthy and use the health care system sparingly so you might consider plan A the Health Savings Account. Can you afford to fund the HSA? If so you might consider an even higher deductible to save on the monthly premium.

You say that plan B is a PPO but it is not eligible for an HSA. Plan B must have somewhat more comprehensive coverage that eliminate its eligibility from use as a High Deductible Health Plan (HDHP) such as the 50% prescription drug benefit.

As far as the dental is concerned I usually find that in the long haul the insurance company comes out ahead. You might get some initial benefits but because of waiting periods, coinsurance and annual benefit limits it is hard to break even over many years of paying premiums.

If your younger, healthy and can afford to fund an HSA that would be the approach I would take.

Celtic and Humana both have a pretty good reputation for customer service and they both maintain fairly strong A.M. Best financial strength ratings.

One thing that separates Humana from Celtic is that Humana is known for having a stronger network than Celtic. This depends a lot on what area of the country you live in and what Doctors and medical facilities you prefer but generally speaking Humana is a little bit more well known and accepted by more health care providers.

One thing that you may want to do is use the provider look up tools on the various insurer's websites to check and see if your preferred Doctor or hospital accepts their insurance coverage.
