Depends on company & insurance carrier policies.

Also, COBRA is not mandatory for EVERY employer, it is based on the size of the business and the insurance coverage. My employees have NO COBRA options.

By law if you qualify for COBRA you will receive an information letter in the mail within 30 days.

I don't think it is accurate, at least as far as a direct connection to the new health care bill is concerned. Discussion of taxing employer provided health insurance is focused on so-called Cadillac plans, and taxation there is deferred several years. You would have to post the exact wording of the e-mail (company name left out, of course) to let someone figure it out.

That's totally fair - in fact, dirt cheap! (My insurance is $400 a month just for ME. Many family plans are in the neighborhood of twice that.) Most companies do ask for employee contribution - some have the employee pay it all. In today's world, it's VERY rare to not have employees contribute something.

You need a written section 125 premium only plan document. After you have this set up the payroll for pretax earnings and deduction. This is should on the W-2 form in box 14.
