plastic surgery by far, most of it is not paid for by insurance and there is a high demand in today's society, It is a mostly "cash" business, people pay up front and you will almost always get paid, also you do not really have to be on call, but watch out for lawsuits.

Another high paying medical career is orthopedic surgery, they make pretty good money too

An RN might be a good choice for you, if you can stand dealing with blood, poop, and sputum.
You can alway take out student loans while you are in school. If you aren't well off financially, you can also look into grants.
