I would try to get some hands on experience first. You can check with your local hospitals to see if they have a career day where you follow someone around. Maybe you could volunteer. You're saying you want to be a doctor, but shouldn't you make sure first? What if you faint at the sight of blood?
Why not talk to your primary care physician and see what he/she thinks and any advice they can give. I think talking face to face with someone to decide the next 8 years of school of your life is important.
Make sure you can afford all the schooling etc. too.

First things first, "how am I supposed to choose between two things that I want?" This people, is called LIFE. There isn't ever a clear-cut decision. There is always going to be sacrifice. With everything we choose.

Becoming a MD is a smart choice. There is a need for doctors and you can specialize in so many things. It's challenging, there's job security, and you'll be making a difference. If the debt or amount of school doesn't bother you, then this would be a great idea. The fact is though, you'll be in a lot of debt. Yes you'll be making a lot of money, but you'll be paying off all your debt for a VERY long time.
Nursing, this is what I'm studying. Why? Because I want a family life, I want to make a difference, I don't want enormous amounts of debt, and I just know it's for me. However, the fact is, right now there is NOT a nursing shortage. And that will change probably in years to come but right now, it's not easy at all for a student who's just graduated.

You have a need for self expression? Do creative things on your down time. Volunteer. The fact is, becoming a chorous teacher isn't something that's in demand. Neither is creative writing. If you love it you love it.
Switching careers 10 years later is a very STUPID choice in my opinion. If you're an MD and you switch over to a chorous teacher I PROMISE YOU that you will never ever pay off your debt.

This is a reality. You're gonna have to make a very important choice. Though like I said you don't have to sacrifice your creativity.

These are pharmacology/research careers. You would need either a PhD in a related chemical pharmacology science or a MD/PhD in the same vein. First, however, you need to get into a good undergrad university. Look into one that is strong in physiological/pharmacological research.

I'm a nurse and I can tell you, the biggest fields to specialize in now are Infectious disease with a specialty in HIV and AIDS, Neurosurgery, and Rheumatology. Cardiologists, Oncologists and Nephrologists are a dime a dozen. You want to specialize in something that most Drs. don't go into, so you are in demand.

Another tid bit of advise to think about. So many Drs.are going out of business these days who have their own offices. HMO's pay you little reimbursement, PPO's the same and very few can afford a really good policy that will pay you what you're worth. Many Drs. are now working for hospitals like Kaiser and VA hospitals, who pay your salary, give you your benefits and you don't have to worry about paying employees, getting them insurance and all those things. You have time to decide, but these are important things to think about.
