She can do both. Sims aren't limited to only one career. As long as she gets some downtime things will be fine. Also she can still play guitar as a hobby and become a surgeon. I say wait and see which overpowers the other and follow that one. Ex. If she is going through promotions as fast as possible but doesn't get guitar skills that quickly then keep guitar as a hobby. If her guitar skill level is higher than her work level then maybe go ahead and work at the theatre.

It depends what options you chose at the start of the game!
I am flexi-time so have more free time.(ooh it is all so real,fab stuff)

Go into the science career (use a computer for this) and at about lvl 4-6 it should ask if you want to stay in science or go to medical.

In my sims 3 all you had to do was click on the sim in need and it said something like give medical attention. If that doesn't work then that particular game might be messed up.

When your Sim comes home from work, look in their Inventory. There will be a notebook in there. Click on it, and Click:Read. Do this everyday until your Job Performance Increases.
