Physician's Assistant: What it sounds like. It's "higher up" than nursing, and I believe you can prescribe medication (though I may be wrong), but the doctor is still the overall boss.

Nurse Practitioner: If you become an NP, you can see patients, diagnose, and prescribe medication. I think it's like the "highest up" position in nursing.

You could also see if you could go into pediatric oncology as a doctor but working in a clinic or group practice. That way you would have more time for family life. However, during residency you would probably be in a hospital, and I'm not sure if there are private practices for oncology.
