Try this: take a sock and fill it with cooking rice
Tie the end off and microwave for 45 secs -1min on high
The heat may help and you can just lay it on an area that hurts
Just microwave it again as needed
And you are exactly right to worry about the Tylenol messing with your liver.

Is your big toe longer than your other toes? If so, you need to use enough padding on the other toes, so they also bear some of the weight.

Another possibility - is the taper of the box forcing your big toe to sit an angle? That's going to give you a bunion, too. You may need to review your choice of shoe. It may help to use a spacer between your big toe and the next toe.

Finally, make sure the vamp of your shoe isn't too long.

try whatever the doc recommends. i had a non repairable torn meniscus and osteoarthritis. keyhole surgery restored knee function. i was able to walk, and eventually go to the gym to strengthen the knee. within six months i had a total knee replacement. this worked very well. after six weeks of physical therapy, i was back in the gym. my replacement surgery was in 6/09. i was 62 at the time. i,ve never regretted this 2nd surgery for one second.
