There are many kinds of doctors who specialize in sports medicine. Most of the time they are orthopedic surgeons, family physicians or internists who practice primary care (as opposed to hospital medicine).

They are completely different. For example, I am sports medicine, but I am not pre-med. Pre-med simply means you're planning on going into medical school. You can be majoring in business and be pre-med. You can be majoring in communications and be pre-med. I'm sports medicine and pre-vet. This means I will be graduating with a degree in sports medicine, but I plan on going to Vet school afterwards. To be completely honest, if you plan on going to graduate school, it doesn't matter what your degree is in. So to answer your question: no, you do not have to be pre-med in order to major in sports medicine. But I should tell you, if you're doing sports medicine, you will most likely have to take a few pre-med classes (but not all). Some of these include biology, chemistry, and maybe physics (depending on your university requirements). Just be glad you don't need to take the infamous organic chemistry or biochemistry.

Edit: Actually, my first three years of college, I wanted to be a sports medicine physician. There are different types of sports medicine specialists. There are physical therapists (there years of physical therapy school) and there are sports medicine physicians. If you want to be a sports medicine physician, that means you would have to go to medical school, so you would technically be classified as "pre-med." Anything with the word "physician" pretty much means you would have to go to medical school. So you can major in anything you want right now (Engineering, Sports medicine, business, economics) and it would not matter as long as you have the pre-medical requirements. Most people major in science fields just because it prepares them better for medical school, but it really is up to you. So you would do four years of undergraduate, then you would apply to medical school. I believe to be a sports medicine physician, you need three years of residency (residency is where doctors pretty much go their separate ways and start specializing in their specific area of interest). So if you want to be a sports medicine physician, you're looking at 4 years of undergrad + 4 years of medical school + 2-3 years of residency and fellowship. It's an incredibly long process but it's very rewarding. Plus, the salary is usually around 250K so that's always a bonus.

Edit 2: This is my advice: As medical school pre-reqs, you will generally need 8 hours of biology, 18 hours of chemistry (organic and inorganic), 8 hours of physics, 6 hours english, and a set amount of electives. Take these, and major in something science related. Sports medicine is fine. The reason I say this is because you will get a feel for how science classes are, and also because most of the time, the classes needed for medical school are also needed for your degree, so you won't end up taking too many extra classes.
