You really should consult your pediatrician about these concerns. There are obviously some underlying behavior issues here. He isn't potty trained at 4, he's still throwing terrible tantrums and is acting silly at times to avoid paying attention. He's deliberately irritating his siblings to possibly get attention or feel superior.

The baby talk is very inappropriate for his age. My 3 year old started doing that when saying her ABCs, but has recently stopped. She's simply trying to get attention and annoy her brother. Hissing and sniffing is very age inappropriate as well unless he's pretending to be something, which doesn't sound like he is.

As for the stroller, I would stop using the single one. You're only instigating your son with that. Use the double stroller always so there is no reason for him to be jealous.

I would consider ADHD, but I'm no medical expert.

This is EXACTLY like my daughter who is 18. But, I do NOT baby her. They will HAVE to learn to do things on their own, daddy won't be around forever.
She was also born with very low muscle tone, and a twisted cerebellum.

Look into 'executive dysfunction'. This is what she has and describes her to a T. The problem is, there is no 'cure' for this and a LOT of work has to be done to help people like this.

They CAN be better, my daughter surprised me SO MUCH because she has gotten her first job at a grocery store bakery and has been there 2 months. I cried about this, being so proud of her. I just always thought she couldnt' do any job.

Your cousin sounds normal.
Staring at ceiling fans is not a sign of autism, how absurd. Where did you hear such a thing? I've never seen a baby _not_ stare a fan when one is present.
His interactions and stimulation should be from people and fun objects, not from the television. Turn the box off and read to him, play with him, talk to him.
He is a normal baby, just see to his needs as he makes you aware of them.
