I am not sure it matters as long as you get in the prerequisites for the OT school that you plan to attend. That school's web site should have its admission criteria, including necessary pre-requisites.

Biology or psychology might be good majors. Human biology would be good if your college offers that major.

If you mean a master's degree in occupational therapy then probably around 2 or 3 years, depending on the school.
And what do you mean you have to take the prerequisites? The only prerequisite for a master's degree is a bachelor's degree, which you'll have.
But look through whatever grad school you plan on going to and see how long it takes to complete the program and what the requirements are for entrance into the program.
They should have course catalog on their website.

I think OT will go the way of PT just as they did with the masters degree. There is a lot of information to cram into a 2 year graduate degree. OT as in PT needs to learn more and more information such as evidence based medicine. DPT graduates are more prepared with more clinical time than an MPT. OTD will be the degree of the future. Unfortanely the salaries have not improved with the degree but I think they will

It's not clear from your question whether you're looking for the cost of graduate school only - or the overall cost from your freshman year through graduate shool.

Colleges all have Websites with pages of information under tuition and fees or financial aid.

The majority of the curricula are, more or less, like this one. I believe you must have a BS/BA.

Occupational therapy is a graduate level program. I am not sure any US colleges offer a bachelor's level OT program that is accredited, just OTA programs. See links below.

Traditionally kinesiology was the "pre-OT major" but today almost every graduate OT program states that any major is acceptable so long as there is sufficient science preparation. English would probably be just fine with most programs as long as you had the science. Some schools still offer a Bachelor of Science in Occupational Therapy which can no longer lead to licensure but can only serve as a prerequisite to an MOT or MA in OT.
