I live in Arlington, Texas right outside of Forth Worth and I have a father who has gotten his doctorate in pharmacy. According to him there are many good selections when it comes to pharmacy programs around Texas, here are the ones you should stick to if you want to stick close to home:

Texas Southern University, UT Austin, Houston University, and Texas Tech University Health Science Center at Amarillo

Could be anything from accidentally dropping a script in the trash to missing a script ( for whatever reason ) and not having it ready for the customer/patient on time. While scripts are involved those errors are not directly related to the actual filling of the script.
