Gait Training Physical Therapy

Douglas Said:

Hi. Any tips on foot physical therapy for any of these; increase ROM, strengthen atrophied muscles, gait train

We Answered:

The physical therapist has all the nice tools and equipment to help strengthen your muscles. For atrophied muscles, using it normally will help get the muscle tone back. Other exercises to do would be to use a theraband to help with resistance so you can strengthen the muscles in the ankle by doing it up and down, circles, and doing the alphabet with your ankle. To decrease swelling, contrast bathing really helps. How do you do that? Take 2 bins, one with cold water (4-5 ice cubes) and one with warm water (shower warm - not boiling). Do your exercises first in cold water for 1 min, then warm 3 min, and then back to cold for 1 min. All of this will help, but if it doesn't improve - get a referral to your nearest physical therapist. Hope this helps!

Anyone with infection control experience out there?

We Answered:

If I were your boss, I would be more worried about your elderly patients coming into contact with you. You state you have a drug resistant strain of mrsa and it is still active. While you may not have an open wound, you still carry the bacteria on your skin, in your nasal cavaties, etc. Since you work with the infirmed and the elderly, you are placing them at risk for infection. The protection your boss should require is gloves, mask, etc. and that should be provided to you to make sure you do not infect those your are trying to help. Most of the elderly who end up in hospitals get infections from health care workers.

I understand that you need to work, but you and your boss are responsible to protect your patients from disease. Your view that you may be infected by one of your charges is selfish and misguided. This is probably not what you wanted to hear. As a long time (now retired) medical professional, I would probably have you work in an area with little exposure to the public, especially the elderly and infirmed, until your infection is cleared up. Good luck to you and I hope they find an antibiotic that works for you.

I have to see Physical Therapy for GAIT Training cuz i experience Vertigo, what is GAIT Training?

We Answered:

People may see PT for vertigo for two major reasons: BPPV (benign positional paroxysmal vertigo) or a central lesion (ie, you've had a stroke, etc) that has affected the brain.

In BPPV (the most common reason people see a PT for vertigo, it involves a series of maneuvers to reposition an otolith or "otoconia". This is a little particle in the inner ear that can become disloged and caused the vertigo (it has nothing to do with ear infection). It's a relatively simple procedure and usually resolves in one visit.

In central lesions, it's a matter of graded exposure to the stimulus to gradually decrease the sensitivity. It takes longer for this type of vertigo to resolve.

Part of the PT evaluation will be to determine if it looks like BPPV or a central lesion. '

In either case (and now getting to your real question), gait training means restoring the ability to walk without loss of balance.

Good luck

She can't legally require you not to wear personal protective equipment. You can get your own masks too. Is she the owner of the assisted living facility or is it a larger organization where she has a boss? I have a feeling you could talk to OSHA and they could get into trouble with the government.
