The minimum requirement is usually that you have a high school diploma and have taken high school level physics and biology and have a C average. It might vary little between programs.

After that, you can apply to a PTA program.

Brown Mackie College - South Bend (A1, D2, F1, PR)
Ivy Tech Community College Northwest (D1, F1, PU)
Ivy Tech Community College of Indiana (D1, F1, PU)
University of Evansville (D1, E5, F1, PR)
University of Indianapolis (D1, E5, F5, PR)
University of Saint Francis (D1, F1, PR)
Vincennes University (D1, F1, PU)

It's usually a two year degree and then you sit for a licensure exam where required.
