Do your best not to use that injured arm of yours that often,especially for vigorous activity. The main aim of the exercises is to strengthen the muscle around your joints so that it could be much stronger. I would say learn to live with it as this is what dislocation is about- occurring time and time again. There is no total cure unless you go for surgery. Use the other arm instead, dude.

put the band opposite the direction you want to go;; band needs to be stabilized by someone else holding it or tie it on something;; suggest:: going back to the facility & asking for an ex sheet, hopefully the same therapist will be there;; don't think they can reinstruct you unless you get a script from the doc, maybe one visit, but it NEEDS to be medically necessary;; another suggest:: they sell tbands in the drugstores now, they may have basic exercises in the box... good luck!!

If you can't get anymore force by a tighter band, then the only other thing you can do is ensure your projectile is released as close to theta = 45 degrees with respect to the horizontal as possible. That is the angle for the max range at any given launch velocity V.

The idealized range equation is R = (V^2/g) sin(2theta), where V is the launch velocity that depends on how much potential energy you can put into that band by stretching it as far as it can go. g is the gravity field.

You should have called you PT after the agonizing run, he or she could have advised you. I would suggest at least one more visit to your PT for advice and an assessment on what is happening. The PT can help plan out a treatment plan that may include your orthopedist.

This is a common place for the shoulder to refer pain...especially the rotator cuff. Even though you didn't do anything to strenuous, it could be that this was more resistance than it was used to...especially if you increased the repetition, weight or if it has been awhile since you did the exercises. It's OK for it to be a little sore the next day for a few hours, but if its much more than that, then perhaps your routine is a little too strong, and I would decrease the intensity.

If it's still happening, you may have an underlying rotator cuff pathology and I would follow up with medical care.

Physical therapy is not a cook book approach--meaning because someone has pain in the X does not mean we give them exercise set Y. Each physical therapy session is highly individualistic and determined based on the results of your specific evaluation. What I might give one person for knee pain might be totally inappropriate for another. Sorry I cannot be of more help, but it would be negligent of anyone to try to advise you over the internet.
