Perry Said:

Who may perform physical therapy evaluation? May a physical therapist (not an MD) perform it?

We Answered:

a doctor will initially evaluate you to see if you are a good candidate for physical therapy - if the doctor decdes that you would benefit from therapy, he will write an order for a physical therapist to do a full evaluation of the patient. this is a very detailed evaluation that includes many steps. the doctor him or herself will not perform the detailed evaluation but rather use his or her judgement to decide whether the patient will benefit from therapy or not.

Arlene Said:

what is the standard physical therapy initial evaluation format?

We Answered:

Even though most people have gotten away from hand written forms, most pre-printed forms still follow the SOAP format and may include any of the following information. (and then some...)

S: (subjective). Chief compliant, pain (words to describe, intensity, location, duration, aggrevating/alleviating factors), self reported functional limiations, past medical history, results of diagnostic testing completed prior to visit, social history, prior level of function, patient goals, etc.
O: (objective)...the results of your exam: AROM/PROM, MMT, gait deviations, posture, balance, tone, muscle length, balance, transfers, standardized testing, etc.
A: (assessment). PT diagnosis, problem list, disabilities, short term goals, long term goals with prognosis
P: (plan): planned theraputic interventions, frequency and duration.

Randall Said:

May a physical therapy provider bill for both (1) evaluation, and (2) procedure for the same session?

We Answered:
