If not better after PT and 2 surgeries...leave it alone for a while. The thought is that frozen shoulder has an auto-immune component to it and the more you aggrevate it, the stronger the auto-immune response. Treat the symptoms conservatively for awhile and let the immune system calm down. I'd also recommend a book called "Treat your own frozen shoulder" by Niel Simon Ascher. While I don't think his functional outcomes are as good as he claims, the symptomatic reduction can be very helpful. It involves some massage type of techniques that can be applied by a second person (spouse, friend, etc).After the pain has calmed down for some time, then try to go back and regain your motion. Possibly with dynamic splinting such as the JAS shoulder splint.

It could be many things...a piece of scar tissue, a small tear in the labrum (the cartilagenous ring that surrounds the "socket"), an airbubble, a rough spot on a tendon...most popping noises are benign...meaning they are nothing to worry about. I've used the JAS splint on patients as oppsed to the dyna splint...did this after a course of failed PT. If you are having to resort to this, it's usually a last ditch effort before surgical manipulation
