It really depends on the insurance company. I'd call your insurance and ask them specifically about the appeals process. Your therapist and/or their care manager, if they have one, as well as your physician may need to provide letters of medical necessity.

I will give you a piece of advice..I broke my ankle and was going through therapy....well little did I know the insurance was not picking up the therapy bill I had to pay for this out of my pocket...$4,000 money that I had to take out of savings. So make sure you check with the women in the office at the clinic....my were young, silly and to busy screwing around.

I'm assuming you mean malpractice. Yes, most employers do provide their employees with malpractice insurance. Yet, it would be important to know what limitations the policy has on aggregate and per occurance claims.

Small, private clinics might not provide you with malpractice insurance and therefore, one would need to purchase their own.

However, malpractice insurance for PTs is very inexpensive since they are considered a "low risk" group. It will also vary where you live.

I pay $134 a year for a private policy above and beyond my workplace coverage.
