I highly doubt it. You're just going to do exercises, et cetera, not have blood work done or anything. A physical therapist has no reason (and probably not even the equipment) to collect a sample and analyze it. I wouldn't worry about it, but to be safe, you should probably give up smoking altogether.

No since PT is a graduate degree. You probably need just the general SAT to get into an undergrad program. Once in undergrad, you need to take one year of gen chem, gen physics, gen bio, A and P, then math potentially up to calc, psy, Then take the GRE and apply for grad school

It's a fairly large component. Like GPA, there are certain standards set by the school (usually at least 800 to 1000), but most schools prefer to see a higher score.
