Your back pain is caused by pinched muscles there and to get rid of the pain you have to free up the muscles, something that p.t. or the pilates won't do for you. Here's how to free up your back muscles to get rid of the pains:
Back:
(do from a sitting position)
Place your left hand on your left leg next to your body. Place your right hand over your left shoulder, fingers over the back and the palm in the front and firmly pull down on them and hold. After 30 seconds slowly lower your body forward and to the outside of your left leg, keeping your left arm fairly straight as you do. When you reach your lap remain there for another 10 seconds, release the pressure but rest there for another 30 seconds. Then reverse your hand positions and do your right side.
For best results relax your body first by taking a deep breath and exhaling then remain this relaxed.

I forgot to add on my last answer that the MRI will probably come out clean as the tight muscles causing your pains don't show up on any of these machines or tests they have, so to the dr. the pains don't exist, and for the ones that believe you they don't have any way of freeing up the muscles to get rid of the pains for you.

When I read your question, I thought, gosh, did I just ask this?? I have a bulging discs in my lumbar (4- 5) region, also causing sciatica (due to the discs being pressed against the nerve root), which causes the pain to shoot down my left leg, all the way to the foot. Also, it causes pain in the bottom of my feet, and my feet tend to "go to sleep" frequently. It is so severe that I can't hardly get out of bed, and the result is that the pain has almost debilitated my life! I was referred to a neurosurgeon 3 months from the onset (Nov. 06') and now, even after a nerve block, and an EMG (a test that can determine how much nerve damage is present, if so, etc), he is suggesting surgery. I've also been seeing a physical therapist, taking prescription pain medication, to no avail. The best advice I can offer you, is to continue your therapy, and with your level of activity, you may consider slowing down a bit, to see if it helps. I've heard bed rest is recommended, but that has not helped me in the least. If your Doctor starts recommending surgery as well, please get a second opinion, and consider all options before doing so. Sorry that I couldn't be more of assistance, I'm also looking at any options that I can find as well.

I am a nurse and recently started to suffer SI joint pain. I have been going to PT for about 3 months now and have had little relief. I consulted some of my nursing buddies, one of them is married to a chiropractor. Both of them wear the SI joint belt and absolutely rave about it. I am going to see my friend's husband next week to get one myself. I pray that it works as well for me as they say it has for them. I haven't heard much negative about them from others yet either. When I get mine, I will try and repost to give more subjective feedback.

Hey there. Sounds nasty. I don't have an answer to your question but, whoever you do see next, ask them to check you out for piriformis spasm. It *could* cause the pain you are describing while not showing up anything interesting on the scan.
