You have a man who loves, respects and trusts you. I really encourage you to think twice, three, five times before you take this irrevocable step. You will not be able to unring this bell and you will hurt not only yourself, but this man whose only crime has been to love you.

If you're old enough to have been married 15 years, then you're old enough to know that when the fire in a relationship begins to fade, what lies at the root of a marriage is that friendship, respect and commitment nurtured over years. What have you done to re-kindle the fire in your marriage? I'd go quite a distance for my best friend and quite a distance further for "the most amazing man in the world".

I truly encourage you to stop your relationship with this other man that is distracting you from working on and building up your marriage. Who knows how much better your relationship with your husband could be if you were focusing that time and energy on him instead of on bedding a more exciting lover? Doesn't it make sense to make that investment in the relationship where you already have 15 years of equity?

I'm no morality police - Lord knows I've enough glass in my house not to be chucking stones - but if you can go back and read what YOU wrote and still feel justified in what you're doing then I've wasted my time here. You're not a teenager - you must recognize that the thrill of the chase and the salivating anticipation of the new will pass with your lover as well. I would bet good money you'll look at this situation with sadly different eyes a few years down the road and then it will be too late.

In the end, even if you choose to divorce your husband, doesn't a man like the one you describe at least deserve the respect of you ending the relationship in an upright way? Does he really deserve you going behind his back, humiliating him and playing him for the fool while you carry on right under his nose with someone he knows, no less? If this is how you treat your best friend and the most amazing man in the world, I'd hate to see how you treat mere acquaintances. Can you imagine the pain of someone doing that to you? The heartbreak and humiliation you'd feel?

I do hope you'll reconsider not just what you're doing, but how you're doing it.

Four hours of a home exercise program for an orthopedically related injury is ridiculous...forget that the body also needs periods of time to recover an rebuild...otherwise inflammation will start all over again.

2-3 exercises max should be recommended, but they could potentially be applied more frequently throughout the day. In my practice, I focus on the primary issue at hand and generally apply only 1 exercise at a time...but often repetitively throughout the day.

There is no physiological reason why four hours of exercise at day will benefit a patellofemoral problem.

I imgaine your program needs to be revised as I imagine you have many exercises that are duplicating the same muscular responses.

We Answered:

1. Yes, absolutely. Chronic Ankle instability is more of a subjective diagnosis based on your ability to function, but it sounds like your ankle is pretty jacked.

2. An orthopedic foot or sports med specialist can perform a bronstrom repair to re-attach your ATFL and tighten your CFL. While they were in there, they could probably deal with the accessory navicular. It would be a significant surgery and require some serious PT and rest, but it would eventually get you back on your feet. You're talking about roughly 4 weeks of non weight bearing followed by another 4 in the boot. This would probably help with the posteior tibialis tendonitis eventually.

3. Typically when an MRI reports that a ligament is torn, that means it's torn all the way through and basically gone. Your ATFL may scar down to a certain degree, but would always be very loose. I think at a bare minimum, you'd be doing regular ankle exercises, like daily, and wearing a quality brace for many years. While it's not an easy decision, I've seen some good results from the Bronstrom repair, including one girl playing Div. I college volleyball.

5. Once you have a few good sprains, you're always more likely to sprain again. In addition to having very loose ligaments, your peroneal muscles (which are responsible for protecting against inversion sprains) tend to have reduced reaction time and poor proprioception. A lot of the PT exercises they made you do were to re-train your peroneals.

One other thing that stands out in your MRI is the "Mild talonvaicular and naviculocuneiform degenerative changes." This basically states that you're starting to develop some arthritis in the ankle. The fact that you're not hurting right now is good, but that's the sort of injury that catches up to you at 55 or 60 years, sometimes earlier.

I'm assuming you're a younger person so you have to think about the long term health of your ankles. All in all, you might have to transfer your care to an orthopedic foot/ankle surgeon. I don't know what surgical procedures your podiatrist does. You kinda got delt a bad hand, good luck with everything.

The fact that the pain is around the abdomen/belly button area and several hours after meals alludes to a gastric ulcer. Ulcers are commonly caused by Helicobactor pylori present in the stomach. Bismuth compounds or antibiotics, and the other two options are good precautions as well, so it's likely all of the above.
