Sciatica Physical Therapy

Daryl Said:

Does sciatica ever go away on it's own? Anyone ever had it go away with physical therapy alone?

We Answered:

It can go away...and mild cases can be managed with a few simple principles and one or two daily exercises that take less than 30 seconds. The first is posture and body mechanics...if you do not correct these, I can garauntee you that your sciatica will NOT go away.

Most cases of sciatica are thought to be due to an abnormal position of the vertebral disc. Repeated motion testing done by a physical therapist who has undergone MDT training (aka the "McKenzie method") has been show in research to reach a 90% accuracy rate in detecting the presence of "disc derrangement"...therefore, it is very common to not have an MRI before going to PT.

My advise is that you find a provider with MDT experience where the focus is on your specific treatment response to movement. Every case is indidualized and education on prevention of reoccurance is the focus of treatment.

Good luck.

Carrie Said:

Does physical therapy help backs w/sciatica?if you got PT,what did they have u do exercise-wise or stretches?

We Answered:

Physical therapy is effective for most cases of sciatica when the mechanical diagnosis and therapy approach is used.

Generally speaking, general exercise helps many people with sciatica in the long term, but patient-specific unidirectional exercises are more helpful in a quicker amount of time when whe MDT approach is used. It is also safer than general exercise.

The exercises are specific to you and dependent on the results of your examination.

I would highly recommend you see a PT who is credentialed in this. Go to: http://www.mckenziemdt.org

Sciatica not going away after several visits to physical therapy. Now what?!?

We Answered:

First, tell your PT...there are several approaches to treating this type of problem...perhaps you need a different approach. If you haven't done so already, I recommend you see someone with MDT experience. Therse are experts in mechanical back and neck problems.

If you have been receiving MDT and are not getting better (ie, symptoms are not getting out of the leg and moving towards the back) and no movement or position decreases or centralizes you symtoms, then this is a sign of a very signficant disc protrusion and probably warrants an MRI. This is especially true if your symptoms are constant (meaning they never go away, even for a breif moment) and if they go below the knee.

If you would like to persue a PT with MDT experience, I would go to: http://www.mckenziemdt.org
