First you go to school and become a PT
Pass the national exam
get a license
get some experience and make some money
Get incorporated or become a PLLC
Rent a space/buy equipment/ pay for malpractice insurance/workers comp/ and liability insurance
You will also need to get health insurance for yourself and any staff. We are members of the chamber of commerence and get insurance through them.
Then apply to each insurance company to become a provider
There are plenty of regulations
You can outsourse billing but it is probably too expensive when you start.
Then you need to market to get patients
You need to make up forms and come up with a type of medical records. You need to come up with your hippa paperwork. You will probably need to hire/outsourse a person to type your dictation unless you plan to type your own reports.

In this day of reinbursement, it is very difficult to open a practice.

You certainly want some experience, and I would also recommend returning for a business degree.

I would recommend you get experience and network with many physicians during this time. Shoot for adavanced certifications and board specialities. Once you've made a recognizable name for yourself, then open a clinic and let those people with whom you've networked that you are opening your own clinic.

I don't own a clinic myself, but this is what seems most logical to me. I'd estimate it would take you about 10 years to build up a good name and reputation for yourself.

Danielle, this will depend on the location, whether you are purchasing or leasing the building? Are you working solo at first or do you have money in reserves to pay salaries? Are you doing the billing yourself? Is the equipment new or used? Does this include advertising? How long can you operate if it takes 9 months for insurance to pay their bills? Can you survive personally if you don't break even each month? What happens if it takes 6-12 months to get credentialed with an insurance (happens often)? Ball park you could begin for something like that, but I would want a lot of physician support as far as referrals go, you would be buying used equipment and working solo, but could be done. Hope this helps.

I totally agree with the other responders. In my experience with a small medical practice, the order of concerns are:
1. Finances (under capitalization will doom any entrepreneur)
2. Employees
3. Insurance payments and contracts
4. Co pays (payments not insurance nor write offs)
5. Professional colleagues
6. Marketing
Initially your concern will be staying in business. A large salary disbursement to the owner is not likely for some time. Remember that you are at the mercy of the market, not the other way around.

Try PT first. Many doctors are referring patients to Physical Therapy as a much better alternative in more than 70% of their patients.
