I can't help with a specific PT but I just wanted to let you know that you may have trouble finding one with those time constraints.

Most PTs work the earlist 7 or so and the lastest usually around 5. And many (most) do not work on Saturdays.

Also most people with that diagnosis will require PT 3-4 times per week not just 1 time per week.
