Yes, hips shaped oddly can lead to back problems. You need to see a spinal surgeon. This surgeon can give you ideas on what to do to help with pain and correct back problems with surgery and physical therapy. Assuming you live in the U.S. you are legally the copyright holder. If she wants to use images of you for anything that does not fall under fair use, she should have a usage license or usage agreement from you allowing her to do so. Just write something up in plain english that makes it clear to whomever creates or prints the materials that she has your permission to use those photos for that purpose.If you ever want to use the images you take of her, you should get a model release from her. You may use the images according to the terms stated in the model release.
Usage and Releases don't necessarily need to be in writing, but it's a good idea to put them in writing.
