Where are you doing your research that most or all cases ended fatal with spread to the lymph nodes and lungs? Morphea type of basal cell carcinoma is the most likely to recur, but it is very rare for them to metastisize. Between BCC & SCC (SCC being the more dangerous of the two) there are more than 1 million cases each year, yet they account for 0.1% of all deaths due to cancer. Mohs has the highest success rate for preventing recurrence. Your doctor knows it hasn’t spread by the depth of Invasion and the fact that you had clear margins. In the rare cases where these cancers are fatal is usually by direct extension and usually occur on the face.
