I would highly get a second opinion by another orthopedic surgeon from another clinic on what is going on (Do not let your doctor know you are going, tell your surgeon's office you need a copy of your records in hand for your insurance company - then you can bring these with you without alerting your surgeon). Either your surgeon screwed up,Physical Therapist, or you did something you shouldn't have. If it is one of the first 2, you will need that second opinion to sue the pants off of somebody and to get help from your new surgeon. Ask around to find out who you should see, your regular physician could direct you unless you know he is buddy buddy with the original one.

Asker, the problem is with neither but in the fact that your body just doesn't want to respond the way that you want it to. When you were on the operation table the orthopedist checked the range of motion of the joint to make sure that it would move completely in the range of motion that he wanted. After the procedure you were hopefully placed in a constant motion machine that kept the knee moving. Scar tissue happens rather quickly and if it is not kept moving it will form haphazardly. If that happens then the joint will not move completely as the scar tissue will act as a tether and restrict movement. For the therapist's part he or she has to move the knee into extension and flexion as rapidly as possible. Unlike the orthopedist that had the opportunity to do so under anesthesia they have to move the knee with you feeling the pain. So the inhibitory factor was your pain levels and no therapist wants to impart the kind of pain that you would have to undergo in order to get the knee into the range of motion that is needed. I am sure that they tried to get that motion but it is very difficult to get there. It might be necessary for the orthopedist to do what is known as a manipulation under anesthesia. This will mean that you are going to have to undergo more therapy and unfortunately it is going to be painful. The scar tissue is going to have to be stretched and more muscle is going to have to be formed to make the knee more functional. Talk to your therapist about how hard they can push the knee. You are going to have to absorb some strong levels of pain whether the knee is manipulated under anesthesia or not to get the full functional capacity out of it.

You did not say what kind of pain, in the kneecap or in the knee joint behind the kneecap. The joint and ligaments there are one kind of pain and release and the kneecap is another. with a bad kneecap you usually have trouble walking and upstairs even more. If it is in the knee joint you usually have pain while standing or the leg wants to kind of turn on you. One of the ligaments is out of sync with the others and pulls more and gets you a pain when it comes down wrong. Here are both ways, take the one that sounds like what you have:
Knees:
While sitting wrap both hands around one leg right behind the knee. With your fingers find the ligaments behind the knee, two on the inside and two on the outside, only one of them is part way up the outside of your leg. (About one inch or less from the one found inside the knee) Press into them with a good amount of pressure and hold tight. Now relax, take a deep breath and exhale and don’t tense up any part of your body. After on minute slowly slide that foot forward until your leg is fully extended. Remove the pressure but rest your leg there for one minute longer.
Kneecap: (Use a chair that has no cross members on it.)
While sitting start with your leg out in front of you extended out to a comfortable length. Wrap your hands around it so you leave a open working area. Place your thumbs about two inches behind your knee and press into the top middle of your leg hard and hold. Relax. After 30 seconds, slowly slide that leg back until it is pulled up under the chair as far as you can make it go. Then release the pressure but hold your leg there for one minute longer.
