How they do it depends on the underlying problem. If they can do it via laparoscopy rather than laparotomy the effects of surgery are much less profound. If it is done by laparotomy it is a fairly major procedure.Kirk Said:

Infection can set in from hours to days after surgery. Although there may be some religion that prohibits antibiotics I have never heard of one and I work for several hospitals throughout Los Angeles County. Our population is very large and diverse so you can name a country or religion and we are very likely to have someone from that country or belonging to that religion.However, when a patient has made their wishes clear, regardless of the reason, their chart and records are clearly marked and the possible outcomes are clearly explained. A patient can refuse anything they want, but a doctor can also refuse to treat, unless it is an emergent situation in which case they have no choice. A Whipple is a planned procedure, very planned, and never done in an emergency so the doctor would have known the patient’s wishes before they booked OR time. In this case, the resident would not have called the attending, unless he wanted his butt chewed, as there is no question whether or not a patient’s wishes should be followed. They are always followed. A patient with pancreatic cancer may have had a consult with an oncologist before the surgery, but the oncologist would not treat them until after surgery.
A Whipple is a long, complex and major surgery with a national surgical mortality rate of around 18%. With an experienced skilled surgeon the rate drops to about 5%. However, complications arise in 20-35% of the cases. Antibiotics to prevent infection is standard with this procedure and I think you would be hard pressed finding a surgeon willing to do it without antibiotics. The patient is likely to die without them so you are better off not putting them through it. Even under the best of circumstances the likelihood this would cure a pancreatic cancer is slim.
In my opinion, it would be more believable for the patient to refuse blood transfusions as is the practice of Jehovah’s Witnesses and hemorrhage is also a complication of the Whipple. Jehovah’s Witnesses have people designated to work with doctors and hospitals so their members can be treated in a way that is following their beliefs.

EDIT: Inverse Mushroom Cloud is right I missed the part about it being advanced this would also make the surgery pointless. Patients are carefully evaluated before undergoing this procedure and only 5-20% have resectable disease. Also a small hospital in the middle of nowhere would not have residences. If the patient doesn't trust western medicine and trusts only God to heal her, what is doing at a hospital? She should just stay home and pray.

The procedure your father wants carries with it great risks and we are doing our best to find the best surgeons for you rather that turn this into some profit making opportunity. We have rejected two doctors in India for lack of experience and our chief medical officer is currently in India working on this for you including finding you a surgeon in Singapore as an alternative as well. We here at planethospital are also very concerned for your father's well being and you know we are working very hard for you. If you feel otherwise, tell us so that we can satisfy you. the resources going into helping your father should not go in vain so if you have no intent in using us, or just want to pick our brain then we would need to deploy those resources for other clients.
